If you are new to KASA, we will need to see a copy of the birth certificate. You may either scan and e-mail a copy to registrar@kasasoccer.org or send a copy of the player's birth certificate to our address:

Notes about your photo
Please upload a photo of yourself for use on identification badges and cards. Your photo should be a recent likeness (taken within the past 6 months) a head shot, a clear image, in color, and with a full front view of your face. Hats and head coverings are discouraged.
File type must be .JPEG, .JPG, .PNG or .BMP.
File must not exceed 3 Megabytes
